Scout Forms & Information

To join Troop 1015, there are a few forms which need to be filled out from a simple application to various health and medical forms. Troop 1015 provides the forms below as well as links to other scout related sites providing further information about the Boy Scouts of America (BSA). The Elks Lodge 1015 is the sponsor of Troop 1015.

– Initial application containing general information about the scout to be filled and turned into the Scoutmaster, Jeff Silva.

 

Medical Forms

Health/Medical – Class 1 & 2

– Class 2 is required of both scouts and adults under 40 to be completed within the last 3 years.

Health/Medical – Class 3

– All Scouts going on the week-long backpacking high-adventure trip need a Class 3 Form completed within the past 12 months. All adults over the age of forty attending Summer Camp or the backpack need a Class 3 Form completed within the past 12 months.

Order of the Arrow –  Arrowmen are known for maintaining camping traditions and spirit, promoting year-round and long term resident camping, and providing cheerful service to others. Scouting’s national honor society.

 

Merit Badges

The Merit Badge Site – MeritBadge.Org provides resources for TigerWolf, and Bear Cub ScoutsWebelosBoy ScoutsVarsity Scouts, and Venturers.
